These T&Cs apply to the provision of cosmetic services by us (NDC Australia Pty Ltd (ABN 49 162 955 866), our related entity and Smilefund plan provider NDC Service Co Pty Ltd (ABN 11 162 958 072), and our other non-National Dental Care, non-DB Dental branded dental practices) to you.

• This offer is only available to patients of Dr Shi Han at National Dental Care Chadstone.
• You must be 18 years old or older at the time of receiving the cosmetic services and accepting these T&Cs. Should you not be eligible for the cosmetic services or the offers we provide, we will supply details of standard dates for consultation for your consideration in advance of your appointment.
• You agree to be bound by these T&Cs. If you do not agree to these T&Cs, you must let us know immediately and we will not provide any further cosmetic services to you.
• You acknowledge that before you can receive cosmetic services from us, you must register with us as a patient. The registration requires the collection of your personal information, including your name, address, date of birth, a mobile telephone number, an email address, health insurance and medicare details and a credit or debit card number. Further personal information will be collected from you as part of our providing cosmetic services to you.
• These T&Cs incorporate our Privacy Policy (https://www.nationaldentalcare.com.au/privacy), the terms of the practice policy and payment terms on our website (https://www.nationaldentalcare.com.au/terms), any consultation treatment letter, and quotations provided by us and accepted by you.
• You need to pay $95 upfront for cosmetic services consultation. While your suitability for the services will be subject to our dentist’s consultation, if you choose to proceed with any of the following treatments after the consultation: Correcting and contouring lips, Enhancing and rejuvenating facial appearance complete and partial packages, Angel Aligners, TMJ and Gummy Smile treatments by Dr Shi Han Su at National Dental Care Chadstone, you will receive a $95 rebate for the initial consultation. You can only redeem this offer for one consultation.
• Our cosmetic services are provided by a qualified practitioner together with the assistance of suitably trained staff. The practitioner conducts cosmetic consultations by:
  • discussing your goals; examining you; making enquiries and taking your cosmetic history; evaluating your goals to consider whether treatment options may be suitable for you; and if any treatment options are deemed to be suitable for you, inform you about them, discuss risks and payment options and provide you with a quote containing an estimated cost for the treatment option.
• You are free to choose whether you wish to proceed with the service following the consultation. While we will undertake best endeavors to support you to achieve your desired results, individual responses and individual results vary. There is no guarantee that any desired outcome will be achieved. Outcomes experienced by one person do not necessarily reflect the outcomes that other people may experience, as results may vary due to many factors, including the individual’s genetics, diet and exercise.
• Subject to our dentist’s assessment of your treatment results, we will provide you with free review and enhancement services within 2 weeks following the completion of your treatment.
• If you have private health insurance, it is your responsibility to check with your health fund which portion, if any, of your fee is covered. While we accept all major health funds, not all health funds offer the same level of benefits and your level of cover will affect your “out-of-pocket” costs. In addition, many of our practices are classified as preferred providers for selected health funds. This allows our patients to gain access to additional exclusive benefits, depending on their health fund and level of health cover.
• Angel aligners are not available for purchase by the general public. It will only be offered following the consultations set out in this set of T&Cs.
• The effectiveness of any consultation is dependent on complete and truthful answers to all questions asked to assess the appropriateness of treatments and medications for you. You understand and agree that in any consultation with our practitioner, you must: answer all questions asked of you; be accurate and fulsome in your answers; and report any new (where applicable, since the last consultation with our practitioner) symptoms or worsening of condition.

You must also ensure that you follow any instructions we give you in relation to any planned or completed procedure as failing to follow instructions may reduce the effectiveness of the procedure and could cause complications.

• We will comply with all our obligations under the Australian Consumer Law in relation to the cosmetic services provided to you, including complying with all mandatory requirements regarding consumer guarantees.
• Subject to the consumer rights and warranties described above, and to the extent permitted by applicable law, we, our employees, contractors and agents will not be liable to you or any third party whether in contract, common law, tort, equity, under statute, in restitution or otherwise, in respect of the cosmetic services for: any indirect, consequential, exemplary, punitive or special losses including loss of profits, loss of opportunity and loss of goodwill; or any injury, damage or loss to any person resulting from your actions in connection with our services.
